Consumers applying for Alternative Care or Elderly Waiver services must participate in a MnCHOICES assessment (formerly known as Long Term Care Consultation or LTCC). MnCHOICES is a face-to-face interview by a county public health nurse and/or county social worker with the consumer to assess the person's need for long term care services (in-home, community or nursing home services). One purpose of MnCHOICES is to prevent inappropriate nursing home admissions and promote the use of in-home and community services when appropriate.

Alternative Care is designed for persons age 65 and older who require a nursing home level of care but choose to receive in-home and community services and, if they would move into a nursing home, would be eligible to participate in Medical Assistance (MA) within 180 days of moving in. MA is a state health care program for persons of limited income & assets.
To apply for the Alternative Care program, please call a Long-Term Care Consultation Intake worker in the county where you live. If you are looking for assistance with other services or more information about counties, contact your local county social service office or public health office.

Elderly Waiver is similar to Alternative Care except it is for persons age 65 and older who participate in Medical Assistance.
To qualify, you must:
- Be age 65 or older
- Be eligible for Medical Assistance
- Need nursing home level of care as determined by the Long-Term Care Consultation process
- Need services that Elderly Waiver can provide for less than the cost of care in a nursing home.
